Lebanon Post investigating fatal crash

Lebanon – The Lebanon Post of the Ohio State Highway Patrol is investigating a single-vehicle fatal traffic crash that occurred on Wolf Creek Court in Carlisle.  

At 6:21 a.m. on April 28,  the Warren County Communications Center received a 911 call advising of a serious injury traffic crash on Wolf Creek Court in the village of Carlisle. Troopers from the Lebanon Highway Patrol Post were dispatched to the scene.

The investigation revealed a 2023 Chevrolet Traverse was traveling westbound on Wolf Creek Court, west of Sunset Drive. The vehicle traveled off the left side of the roadway and struck a mailbox. The Traverse re-entered the roadway, struck a parked vehicle, and drove off the right side of the roadway. The vehicle drove between two houses, down an embankment, and struck a tree.

The driver, Pamela Goins, 78, Miamisburg, was removed from the vehicle by a Carlisle Police Officer and witnesses.  CPR was administered until EMS arrived on scene. EMS transported Mrs. Goins to the Atrium Medical Center, where she later succumbed to her injuries.

Troopers were assisted on scene by the Carlisle Police Department, the Warren County Sheriff’s Department, the Carlisle Fire Department, J.E.M.S. EMS, and Steve’s Towing.
