Lebanon Post investigating 2-vehicle fatal crash in Warren County
The Lebanon Post of the Ohio State Highway Patrol is investigating a two-vehicle fatal crash that occurred Monday, May 22 at approximately 8:30 a.m. on Greentree Road near Union Road in Monroe, Warren County.
Preliminary investigation revealed a 2013 Phoenix concrete-mixer, operated by Steven Carpenter, 31, Hamilton, was traveling east on Greentree Road, and a 2003 Toyota Camry, operated by William Mapes, 82, Lebanon, was traveling west on Greentree Road. The Toyota went left of center, striking the Phoenix head-on.
Mapes was transported by ground ambulance to Atrium Medical Center with life-threatening injuries. Mapes later succumbed to fatal injuries as a result of the crash.
Carpenter was not injured as a result of the crash.
The patrol was assisted at the scene by the Warren County Sheriff’s Office, Turtlecreek Township Fire Department & EMS, Warren County Engineer’s Office, Case Towing and Reffitt’s Garage & Towing Service.
The crash remains under investigation.
