Lebanon OSHP Post handling double fatal crash on S.R. 741

The Lebanon Post of the Ohio State Highway Patrol is investigating a double fatal vehicle crash. The crash occurred March 13, at approximately 10 p.m. on S.R. 741 near mile post 6 in Turtlecreek Township, Warren County. 

The preliminary investigation revealed a 2019 Tesla Model 3, operated by Dylan Sougstad, 17, Cincinnati, was traveling south on S.R. 741. A 2020 GMC Terrain, operated by Chastity Smith, 53, Trenton, was traveling north on S.R. 741. The Tesla traveled left of center, striking the GMC. The Tesla then traveled off the west side of the roadway and the GMC traveled off the east side of the roadway. 

Both Sougstad and Smith were pronounced deceased at the scene. 

SR 741 between Hamilton Road and S.R. 63 was shut down for several hours for cleanup and removal. The crash remains under investigation at this time by the Lebanon Post

Troopers were assisted on scene by the Warren County Sheriff’s Office, Turtlecreek Township Fire Department and EMS, ODOT, Warren County Coroner, and Jacob’s Towing.
