Lynchburg-Clay kindergarten registration to be held April 14

Kindergarten registration for students living in the Lynchburg-Clay School District will be held on Wednesday, April 14, 2010 in the Lynchburg-Clay Elementary building. Students with last names beginning with A - L will register from 9 - 11:30 a.m. Students with last names beginning with M - Z will register from 1 - 3 p.m.
In order to enroll, the parents must provide the following:
* An official birth certificate with a state seal. This can be acquired from the health department in the county of your child's birth. A child must be 5 years of age on or before Aug. 1, 2010.
* Immunization records. The state laws of Ohio require that certain immunizations must be completed before the child enters school.
Five doses of Diphtheria/Pertussis/Tetanus (DPT, DtaP, DT), unless the fourth dose is administered after the pupil's 4th birthday. If 4th dose is administered after the 4th birthday, only 4 doses are required.
Four doses of Polio (OVP, IVP), unless the third dose is administered after the pupil's 4th birthday. If 3rd dose is administered after the 4th birthday, only three doses are required.
One dose of Varicella
Two doses of Measles, Mumps, Rubella (MMR)
Three doses of Hepatitis B Vaccine (HBV)
* Your child's Social Security card.
* Proof of residence. Driver's license not acceptable.
* Custody papers if applicable.
During registration, parents/guardians need not have the child present. Parents/guardians will be selecting a time in May, at which time students need to be screened for hearing, speech, vision, and communication. The student will also have a physical and fingerprinting completed.
