Adena names Kittaka chief transformational officer

Adena Health System has named H. Takaji Kittaka, Jr., MD as its Chief Transformational Officer. In this role, Dr. Kittaka will be responsible for overseeing primary care throughout the health system.

The role of the Chief Transformational Officer focuses on working with the Adena Medical Group in adapting to changes that are the result of new care models in the field of health care, many the result of the Affordable Care Act. These include: working to provide optimal quality care at a lower cost to patients and providers; achieving quality results in patient outcomes; and increasing provider accessibility.

Dr. Kittaka also serves as a primary care physician at Adena Family Medicine – Waverly, and has oversight of Adena’s Patient Centered Medical Home (PCMH) initiative. The PCMH program pairs patients with nurse navigators who monitor their condition to control chronic disease with the goal of reducing hospital admissions.

With Adena Health System since August 1999, Dr. Kittaka is certified by the American Board of Family Medicine. He is a graduate of the University of Nevada’s School of Medicine and performed his internship at the Utah Valley Family Practice in Provo, Utah.
Dr. Kittaka and his family reside in Pike County. Two of his five children are graduates of Waverly High School and are currently attending The Ohio State University.
