Adams County Community Foundation supports Hospice of Hope patient care initiative

The Adams County Community Foundation has awarded a grant to Hospice of Hope to support a special patient-centered initiative designed to provide comfort, dignity and meaningful personal connections for hospice patients and their families.
The grant will help fund the creation of personalized “All About Me” boards that will be developed and decorated by Hospice of Hope volunteers for patients receiving hospice care.
Each board will highlight important aspects of the patient’s life, including their personal story, favorite things, family memories, and what matters most to them.
The boards are designed to help caregivers, family members, and visitors better understand and connect with each patient on a personal level while providing individualized attention and compassionate care throughout their journey.
In addition to serving as a communication and memory-sharing tool during hospice care, the boards will remain with families as keepsakes and reminders of their loved one’s life and experiences.
“These boards help celebrate the unique story of each patient while providing comfort and meaningful connections for families during a very important time,” said Michael Parker, Director of Development for Hospice of Hope. “We are grateful to the Adams County Community Foundation and its donors for supporting projects that focus on compassionate, individualized care.”
Linda Stepp, president of the Adams County Community Foundation, said the Foundation is pleased to support programs that directly impact the quality of life for Adams County residents and families.
“Hospice of Hope provides extraordinary care and compassion to families throughout our community,” Stepp said.
“This project reflects the dignity, kindness, and personal attention that hospice care represents, and we are honored to help support it.”
The “All About Me” boards will be used wherever patients call home, including at the Adams County Regional Medical Center Hospice of Hope In-Patient Care Center, and will travel with patients throughout their care journey.
